Project co-financed from European Funds in cooperation with Sir Med Medical Centre.

The aim of the "Day Care Centre for Medical and Nursing Long-term Care for the residents of the Inowrocław sub-region". (DDOM) is to increase the availability of medical and nursing services and support the process of deinstitutionalization of care for dependent persons from the districts of Inowrocław, Mogileń and Żnin.

Within the project participants receive support:

• medical
• psychological
• rehabilitation
• nursing

in Sir Med Medical Center in conditions similar to home or directly at the patient's home.

Assistance is provided once a month/week or daily Monday through Friday from 8:00 a.m. to 4:00 p.m.

As of September 2019, we have supported 38 individuals and their caregivers.
